These Terms of Use govern your access to and use of the Cordi platform operated by Centaur Institute (Ci), the trading name of OrthoSport Victoria Institute Pty Ltd.
Eligibility
Access to Cordi is limited to individuals enrolled in a research program approved by a Human Research Ethics Committee (HREC), and to their authorised clinicians and coaches.
Your Responsibilities
- Provide accurate information when registering and completing assessments
- Keep your login credentials secure and notify Ci of any unauthorised access
- Use the platform only for its intended clinical and research purposes
Our Responsibilities
- Maintain the platform in accordance with applicable Australian health information standards
- Process your personal information in accordance with our Privacy Policy
- Notify you of material changes to these terms
Limitation of Liability
Cordi is a data collection and reporting tool. It does not provide medical advice. Clinical decisions remain the responsibility of your treating clinicians.
